This service is for patients returning for a follow-up appointment to review levels, assess treatment progress, or address any other needs of the patient. To ensure we can review all relevant information during your appointment, please fill out any pertinent forms. If labs are going to be reviewed, they should be drawn minimum of 2 weeks prior to the consultation.
